Animal Hospital is one of the most popular, and, honestly, one of the most stressful games on Roblox right now. We’ve covered a few smaller guides on it, but today, it’s time for a full beginner’s guide. In this Roblox Animal Hospital walkthrough, I’m going to run through everything important in the game, so you’ll know how to survive the worst the bizarre, Animal Crossing-turned-apocalypse horror game has to offer.

Roblox Animal Hospital Walkthrough

We’ll start with the most simple parts and build from there. At the start of a run you’ll only have access to a small number of facilities anyway, so we’ll start with what you get at the very beginning before we cover more complex bits.

The Basics

So, the basics of the Animal Hospital are reasonably straightforward. You are a cartoon animal. You work in a hospital, also for cartoon animals. Some things that are not cartoon animals, but are pretending to be, will try to get in posing as cartoon animals, and you need to turn them away, while treating the cartoon animals. Confused? Basically, every patient that comes through works to a certain pattern of tasks you (or one of the players you’re playing with) need to do. This is how it plays out for a normal patient.

  • Greet them at the Reception desk and take their paperwork.
  • Snap a picture with the camera, enter their details into the computer, and print out an ID at the printer, which you present to the patient.
  • The patient will head to a Treatment Room.
  • Head to the correct Treatment Room.
  • Interact with the patient to take a DNA sample.
  • Put the sample in the Sample Analyzer.
  • Check the Treatment Room computer to diagnose.
  • Check the display screen above the patient to see what the recommended treatment is.
  • Pick up the items needed to treat the patient from the Treatment Area lobby and bring it back to the Treatment Room.
  • Interact with the patient to administer treatment.
  • A timer will start, indicting the time of recovery.
  • Once full recovered, the patient will check out of the hospital.

All very simple…but then you’ll get to…

Anomalies

Screenshot from Animal Hospital showing a player viewing a patient photograph. The NPC, a yellow cat with a frowning space, stands in front of the window. The photograph shows their face as distorted, with a grinning face with teeth out of alignment.

Among your regular (for talking pink cat) clients, you might get a few strange creatures sneaking in, pretending to be patients. Along with your other responsibilities, you need to root out these impostors and bar them from entering your establishment, or there could be consequences.

Signs of anomalous patients include:

  • Hollow, missing eyes.
  • Huge teeth (seen on the CCTV).
  • Erratic body twitching.
  • Photos appearing distorted on check.
  • Appearing as a different species on animal on the CCTV.
  • Appearing in a different pose on the CCTV.
  • Speaking in a deep, gravelly voice.

If the patient appears suspicious in any way, you should press the red button to the left of the desk window to close the shutters. This should cause the patient in question to walk away, and leave you alone. If you let one through they may trigger Monster Events, or start attacking patients. We’ll cover that more in Anomalies Events.

Sanity

Dealing with horrible unnatural things does take a toll on you. As you work, your sanity, shown by the brain-shaped bar in the bottom left of the screen, slowly ticks down. You can lose it even fast if you’re subjected to distressing sights, like entities around the hospital, distorted pictures, or skinwalkers trying to eat people in the waiting room. All of these take a toll. You don’t want your Sanity to hit 0. Luckily, there are a few ways to raise it.

  • Coffee – You’ll find the coffee pot in the front office. Interacting with it gives you a cup of coffee, which will replenish Sanity with each sip you take. Once you’ve taken a cup, it will enter a cooldown as it brews another cup. (For this reason, it’s a good idea to grab a cup of coffee sooner rather than later, as the machine can start brewing again.)
  • Chocolate – This restores a lot of Sanity, though appears to be a Robox-only item.
  • Class Skills – Some Classes (see below) get Sanity boosts from certain tasks, such as Doctors regaining Sanity from treating patients. Some classes, like the Intern, start with bonus Sanity.

Treatments

Screenshot from Animal Hospital showing a patient, a pink cat, lying on a bed in a Treatment Room. The main screen over the bed reads that the patient needs Eyedrops.

Each condition diagnosed through the DNA Analyzer, Heart Scanner, or X Ray Machine will flag up a treatment you need to go fetch. You’ll find them in the hallway area of each Treatment Room block. They are as follows.

  • Thermo – Treats Fevers.
  • Bandages – Treats Bleeding.
  • Medkit – Treats Bruising
  • Herbs – Treats Stomach Ache
  • Eyedrops – Treats Dry Eyes
  • Maple Syrup – Treats Low Blood Sugar
  • Medicine –
  • Cough Syrup – Treats Coughs

The screen in the treatment room will tell you which items to get, through it does make things faster if you know what to grab as soon as you see the symptoms.

X Rays, Scans, Surgeries

Screenshot from Animal Hospital showing a Surgery round. A blue cat NPC lies on an operative table, eyes closed and snoring. A skull icon and countdown appear over the character. Various items like a medkit and some scissors sit on a worktop nearby.

After you’ve made it through your first couple of days, the second set of Treatment Rooms open up. Most of these are pretty similar to the ones you used in the first block, but three are unique and need different tasks from you.

Room 6 – ECG Room

In this room you’ll hook the patient up to a heart monitor to begin diagnosing them.  This should open up a minigame that you need to complete to get the info. The minigame is reasonably simple:

  • You will see the scanner screen in the centre of your view.
  • Circles will expand and shrink on the screen.
  • You need to hit the icons marked with the tapping hand symbol.
  • Avoid hitting the circles with the red skulls.
  • Once you’ve hit enough to fill the progress bar you’ll complete the scan.

Room 7 – X Ray Room

In this room you should find the patient standing inside the X Ray Machine. Like the ECG Room you’ll need to complete a minigame.

  • Under the viewing window into the X Ray Machine are a line of buttons.
  • Buttons will light up in a certain sequence.
  • Copy the sequence and press the correct buttons to launch the scan.

Room 8 – Operating Theater

This large room with the table in the center is the set for the (rather stressful) Surgery round. A patient due for Surgery will lie down on the table. When you interact with the patient, it will trigger the Surgery round.

  • A timer hovers over the patient. It starts at 45 second but counts down throughout the Surgery. If it reaches zero before the Surgery is finished… well the skull icon probably gives you some idea.
  • Different treatment items will flash up on the screen. You will need to fetch them and take them to the patient as quickly as possible. These items are on the worktops around the table.
  • If you complete all the tasks within the time limit, the patient will survive, and begin to recover.

The Shop

After the first shift, you’ll find the Supply Shop open during your shift. This lets you trade in money for upgrades that can improve the performance of the hospital, and keep you going longer. The stock is randomly picked from a pool, so there’s no guarantee what you’ll get, but it can include:

  • Speed boosts to equipment such as computers.
  • Extra reception desks.
  • Weapons like a taser or a gun.

The Shop is only open for a limited period, so if you see something you want, keep an eye on the shop’s timer, and make your purchases ASAP.

The Classes

Screenshot from Animal Hospital showing a the Secret Agent class, a black cat with no visible nose or mouth, and wide yellow eyes.

One important part of Animal Hospital is classes. These all give slightly different stats and special abilities. You can gain XP in each class by using it, and unlock higher tier versions of class abilities. You will need to pay Coins earned in-game (or, in some cases, Robux) to unlock all but the default Intern class.

Intern – Default

  • Medium Sanity
  • Very Low Utility
  • Very Low Starter Money

The default class. Gets bonus Sanity, maybe because they’re the only one who doesn’t have to look at their horrifying face. Are we sure that’s not an anomaly?

Nurse – 20 Coins

  • Very Low Sanity
  • Medium Utility
  • Very Low Starter Money

Slightly more qualified than the Intern, has maybe seen too much though. Can carry extra inventory items.

Secretary – 120 Coins

  • Medium Sanity
  • Low Utility
  • Low Starter Money

Not too bad as a low level all-rounder. Gains Sanity from doing patient check-ins.

Paramedic – 250 Coins

  • Very Low Sanity
  • High Utility
  • Very Low Starter Money

This deer has seen some stuff, but is great at what they do. Starts with speed-bosting cola on hand.

Psychologist – 500 Coins

  • Very High Sanity
  • Low Utility
  • Very Low Starter Money

Armed with an understanding of the workings of the mind, and a majestic mustache. Negative or positive Sanity effects are enhanced, though at later levels you will ignore a certain number of Sanity-dropping events.

Doctor –  900 Coins

  • High Sanity
  • Medium Utility
  • Very Low Starter Money

Gets a white coat and an air of authority. Thought Doctors were better paid than this, though. Gains extra Sanity from healing patients.

Security 1,250 Coins

  • Very Low Sanity
  • High Utility
  • Low Starter Money

Has an official security hat, and maybe more threateningly, comes with a taser.

Head Nurse –  Robux Only

  • Medium Sanity
  • High Utility
  • Medium Starter Money

Pretty much an upgraded version of the Nurse class.

Surgeon – 2,500 Coins

  • High Sanity
  • Medium High Utility
  • Medium Starter Money

Upgrade from the Doctor, this class regains sanity after treating patients.

Secret Agent  – Robux Only

  • Medium High Sanity
  • Very High Utility
  • High Starter Money

How did this guy get in here? And why does he have a GUN?

Anomalous Events

Screenshot from Animal Hospital showing a Death Ritual event. It shows the inside of a Treatment Room, lit with an eerie red light. The screen on the wall appears corrupted with red writing and symbols scrolling across it. The patient, a yellow bear NPC with a bandage around their head, is floating above the bed, with wide yellow eyes and a creepy grin. Several candle flames can be seen at the bottom.

Alongside treating the patients, you will have more unexpected happenings to attend to as you continue your career as doctor/nurse/chief surgeon/receptionist/janitor/security guard/all of the above at Animal Hospital, and not handling them promptly can have terrible consequences. Some of these will conjure up notifications on the bottom of your screen, complete with countdown to deal with it. Some we ran into are:

A Skinwalker Is Eating A Patient!

Looks like you didn’t keep all the anomalies out today. You’ll find an NPC with a big toothy mouth snapping at someone in the waiting room. You can take care of this by grabbing your trusty taser from the office and giving the monster a zap of justice. This should cause the Skinwalker to run away.

Don’t Look Up

Sometimes the ceiling gets oddly… drippy. Also full of meat and eyes. When you step into a room with this, a message flashes on the bottom of your screen. ‘Don’t look up.’ Do exactly what that says and pretend the ceiling is totally normal.

Monster Event

Sometimes after conducting a Surgery, things get a lot worse, and a monster bursts out of the patient like a scene from The Thing. This triggers a second Surgery Round where you’ll need to fix this and deal with the monsters before the countdown ends. If you can do that, the patient should pull through.

Head-Banger

Sometimes an unsettling NPC with hollow eyes will shuffle over to the front desk, and start to bang their head on the glass. This isn’t harmful exactly, but it will stop you from closing the shutters, which you may need to do. To get rid of this unwanted guest, interact with them to make them move on, but do expect a jumpscare and the sanity drop to match.

Death Ritual

While going about your job you may see a notification that someone triggered a Death Ritual in a certain room, along with a countdown. On entering, you’ll see the patient floating above their bed, and some sinister ritual circle painted on the floor. That’s not good. To put a stop to this, you need to interact with every candle in the room to extinguish them.

A Patient Is On Fire!

How did this happen? Either way, you need to take care of this ASAP. Grab the Fire Extinguisher, found next to The Shop, and spray it over this patient in question.

Entities

Aside from Skinwalkers, you may see other entities lurking around the hospital.

  • Camouflaged entities that blend into walls. They give a sanity penalty on discovery.
  • The Stalker, a large shady rabbit with elongated limbs.

Special Shifts

You may also be called for Special Shifts. These usually focus around an unexpected event, like an anomaly attack causing mass casualties. These will happen largely without warning, and often mean a large influx of patients at once, sometimes skipping the front desk (allowing anomalies through). These tend to be more chaotic and tricky than your average shift, but they will award you with extra overtime pay at the end of the shift.

Tips

So, now you have a rough idea of everything you need to do, here are a few tips to help you juggle all the responsibilities.

  • If you’re in Multiplayer, delegate jobs. Don’t be there trying to fill every staff position, it just gets messy. Have some players manning the desk, while others handle the treatments. Everyone can jump in with events like the Mass Casualty events, but it does pay to specialize, especially if you’re in a class that does well.
  • Deal with timed events first. Most patients can wait for a while, but if something has a timer pop up at the bottom of the screen, you should probably drop everything to deal with it.
  • Drink Coffee ASAP. The earlier you drink it, the quicker another pot brews, the better your sanity level.
