Animal Hospital has some funny little quirks, including some absurd ways to handle infiltrating anomalies. I wanted to compile all the ways I know how to deal with each Anomal in Animal Hospital, so you can better understand their mechanics and survive a few extra shifts.

Let’s get into it.

How To Deal With Each Anomaly in Animal Hospital

Bed Monster

If you spot an entity under a patient’s bed, don’t step inside the red box, unless you have an offering of an Organ, a Transplant, or Maple Syrup. This will pacify the bed monster, and it will eventually leave on its own without harming the player.

  • Features: Appears under patients’ beds. Is a black mass with two eyes and lunging arms that snatch anything which steps into the red box in front of it.

Hider

The Hider chases the player after being noticed by them, so they need more direct action to disable. You can lure Hiders to the Bed Monster (jump over the bed, and the Hider will walk into the red box), and they will get eaten.

Alternatively, use a fire extinguisher, a gun, or a Taser to destroy them. If a room is on fire, you can also lure the Hider within, and it will burn up in the room. Finally, if you’re without items, jump onto higher surfaces such as the item cupboards and wait for the Hider to leave on its own.

  • Features: A mimic-like animal that blends into walls, only noticeable through its uncanny smile and wide eyes. If looked at, it will chase the player.
    • Note: If the player holds an item in their hands when attacked by a Hider, the Hider will steal the item instead of harming the player.

Stalker

A long, thin entity that curves unnaturally around corners to spook the player into losing sanity. Looking into its eyes will cause the player to lose 10 Sanity. However, looking at it through a glass (such as the reception area) will not trigger the sanity loss.

  • Features: Lanky and tall, this entity hugs room corners to spike the nerves of unsuspecting players. Looking at its feet or away will negate all effects. This entity has a noise queue and triggers light flickering before it creeps from around a corner.

Mass With Eyes

Oozing from the ceiling, this entity makes the player lose Sanity when making direct eye contact. It doesn’t move or attack the player, but it can be disabled if using eye drops. However, to use eye drops, you need to look up, sacrificing some sanity anyway.

  • Features: A giant red mass with far too many eyes that carefully watch patients in their room and the nimble assistants that aid them.

Ghost

Ghosts are invisible unless you reveal their location using a fire extinguisher. When visible, use a gun or taser to dispel it permanently.

  • Features: A floating white husk of a recently deceased. The Ghost spawns when a patient in the hospital dies and isn’t properly cared for.
    • Note: Throwing patient corpses in the trash can will prevent a Ghost from spawning.

Tentacles

During surgery, some patients will spew purple tendrils that writhe. These don’t harm the patient or player, so to deal with them, you need to continue the surgery as normal. Shooting the tentacles or tasing them will instantly kill the patient.

However, if the patient is a shapeshifter, shooting them during surgery won’t kill them, as the NPC is an anomaly.

  • Features: Wriggly purple tentacles that spike up from the patient’s body.
    • Note: You can stand on the surgical table in the middle of the room and access all equipment easily without needing to walk around.

Camera Entity

When checking the cameras, there is a chance to see an all-black creature staring into the camera that isn’t actually there. Once it notices it is being watched, the camera will begin to zoom in and trigger a jumpscare with -20 Sanity if the zoom completes. To avoid this monster, switch cameras or exit them completely.

  • Features: A black monster with glowing red eyes and yellowing, jagged teeth. This entity always looks into the camera.

Shapeshifter

They look fine until they’re not. Shapeshifters are sneaky and aggressive, infiltrating hospitals by changing small features of themselves during check-in. If they enter the hospital, use a gun or taser to kill them. If you’re without weapons, giving them the wrong medicine during treatment will also automatically kill them.

  • Features: Once revealed, the shapeshifter has an agape mouth with rows of sharp teeth and a protruding, pointed tongue.

Other Anomaly Types

Some anomalies only ever make it to reception due to their poor disguise or motivations. This includes:

  • Realistic face: Large black and white eyes with notable eye bags, and a wide, drawn-on smile that fades after a light tasering.
  • Three eyes: This anomaly has three slightly glowing eyes.
  • Crooked face: One eye rests notably higher on the anomaly’s face than the other, paired with a crooked and sharp smile.
  • X-Ray face: Small but wide circular eyes and realistic teeth. This anomaly type has a face similar to that of a human X-Ray.
  • Staring: Their face follows the player, no matter the distance. This feature is sometimes only noticeable through security camera checks.
  • Hollow Eyes: Wide and long black eyes with a hollow frown. This anomaly type will always be hunched and twitching, and attempting to heal one guarantees a death ritual.
  • Twitching: Their body, especially their neck, twitches and snaps unnaturally.
  • All Black: The patient appears entirely black with no notable features or assets. Sometimes only visible on cameras.
  • Black Eyes: A black box with realistic eye textures over the patient’s real eyes.
  • Warped Body: The patient has a very crooked posture with elongated limbs and warped assets.

A general rule of thumb is that any NPC hunched over with poor posture is most likely an anomaly. Additionally, you always need to take note of pictures and compare them to the subject before you. Small things can change, indicating you’re in the presence of an anomaly. This includes:

  • Eye appearance changing.
  • Ear shape changing.
  • Ear colour changing.
  • Mouth shape changing.
  • Static photo.
  • Cursed photos.
    • Major details changing, such as a creepy, realistic face.
