When you unlock a new tool, the game will tell you that you have to watch a video or lose it forever. That isn’t the case. Clean until you can tap
You’ll want to keep cleaning the teeth until the hand icon appears above the button that lets you move on. That’s when you know you’ve done enough that you can move on to the next part of the procedure. Keep cleaning until it’s there and then head on to the next challenge.
Play the VIP levels
When VIPs appear you have to watch a video to perform their procedure. It’s worth it, because you’ll get double the rewards if you manage to sort their teeth out. These challenges aren’t any more difficult, so it’s worth taking them on to get the extra cash.

When you’re fitting the braces, you’ll want to push down on the screen until some stars pop up around the patient’s teeth. That’s when you know when you’re done. Don’t worry about hurting them, you can hold on for as long as you want once the braces are properly fitted and nothing bad will happen.
